Allergic reactions during anesthesia: diagnosis and treatment

چکیده
The term “anaphylaxis” is composed by “a na”, meaning “not or contrary to” and “phylaxis”, meaning “protection”. Thus repeated exposure to a toxin may cause harm instead of prophylaxis or immunization. An anaphylactic reaction or anaphylaxis is an immediate type hypersensitivity reaction to a substance or drug, causing acute life-threatening symptoms in two or more organ systems. Immunoglobulin IgE antibodies are involved. The reaction is independent of the pharmacological actions and the dose of the substance or drug. An anaphylactoid reaction is clinically indistinguishable from an anaphylactic reaction, but is not mediated by IgE antibodies. As a consequence a definite diagnosis can only be made after investigation (7,17,18,23,25,30,34,38). Before investigation of a reaction the term “suspected anaphylactic reaction” should be used (36-38). However, in the literature there is a lot of confusion about the terminology (28) while during last years it has been suggested to abandon the term anaphylactoid. During anesthesia many different drugs are used in rapid succession: not only anesthetics, but also antibiotics, fluids, nonsteroidal anti-inflammatory drugs and other compounds (e.g. disinfectants, latex etc.). Most of them are given intravenously and in bolus, bypassing the body’s primary immune filters and presenting high concentrations of antigen directly to the mast cells and basophils. So it is difficult to say which drug caused the suspected reaction or that the reaction was the result from the additive side effects of several drugs injected simultaneously (40).

Diagnostic value of histamine and tryptase concentrations in severe anaphylaxis with shock or cardiac arrest during anesthesia.
BACKGROUND The diagnosis of acute life-threatening allergic reactions during anesthesia relies on clinical signs, histamine and/or tryptase measurements, and allergic testing. In patients who die after the reaction, skin tests cannot be performed, and the effect of resuscitation manoeuvres on mediator concentrations is unknown. The authors compared plasma histamine and tryptase concentrations i...

A dentist's ability to safely administer regional anesthesia is essential for dental practice. Local anesthetic solutions used in the United States for dental anesthesia are formulated with several components. The contents of a standard local anesthetic cartridge may include an amide or ester local anesthetic drug, an adrenergic vasoconstrictor, and an antioxidant. In susceptible patients, any ...

متن کاملEarly skin testing is effective for diagnosis of hypersensitivity reactions occurring during anesthesia.
Allergic skin tests have to be performed 4-6 weeks after an allergic anesthetic reaction. Patients with allergic reactions during anesthesia were prospectively included (n = 44). Skin tests were performed in two stages: (i) Stage 1 (S1), 0-4 days after the reaction; and (ii) Stage 2 (S2), 4-8 weeks after. Five (11.5%) surgical procedures were suspended due to the reaction. Positive skin tests w...

متن کاملAllergic reaction to patent blue dye in breast surgery - case report.
We present a case of allergic reaction to patent blue in a patient who underwent excision of sentinel lymph node associated with segmental breast resection. About 20min after the dye injection, the patient developed hypotension (BP=70×30mmHg) associated with increased heart frequency. The patient was treated successfully with decreased inspired fraction of inhaled anesthetic and fluid replaceme...
